The Practice will deal with requests for access to medical records from a patient, representative for the patient, or outside body in accordance to General Data Protection Regulation (GDPR) and the Data Protection Act 2018. If a patient would like to request copies of medical records, they will need to complete a Subject Access Request form, available from Reception or by clicking on the link below. This form will need to be returned to the surgery with the appropriate identification. Under GDPR, the Surgery will have one month to respond to a request. If a request is more complex it may be extended by a further two months (three months in total).
